Why can't I just breathe slowly on my own?
You can, and it works. The issue is doing it. Isn't this just an expensive necklace?

How fast will I feel something?
Most people notice a physical change within 60 to 90 seconds of the first use. The vagus nerve starts slowing your heart rate at around 30 seconds. Do I breathe in or out through it?

Will this work if I have real anxiety, not just stress?
